THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Responsibility

Mobile Rehab LLC is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this notice of our legal duties and privacy practices. We are required to abide by the terms of this notice currently in effect.

2. Uses and Disclosures of PHI

We may use and disclose your health information for the following purposes:

  • Treatment: To provide, coordinate, or manage your physical therapy and related services. This includes disclosure to your referring physician or other healthcare providers.
  • Payment: To bill and collect payment from you, an insurance company, or a third party.
  • Healthcare Operations: For activities necessary to run our business, such as quality assessment, clinical audits, and licensing.
  • As Required by Law: We will disclose PHI when required by federal, state, or local law (e.g., reporting abuse, neglect, or responding to a court order).
  • Communication: We may contact you for appointment reminders or information about treatment alternatives via phone, text, or email.

3. Your Rights Regarding Your PHI

  • Access: You have the right to inspect and copy your medical and billing records.
  • Amendment: You may ask us to correct information that you think is incorrect or incomplete.
  • Accounting of Disclosures: You can request a list of certain disclosures we’ve made of your PHI.
  • Restriction Requests: You have the right to request a limit on how we use or share your PHI. We are not required to agree to your request, except if you pay for a service out-of-pocket in full and ask us not to share that info with your health insurer.
  • Confidential Communications: You can ask us to contact you in a specific way (e.g., home or office phone).

4. Our Mobile Practices & Electronic Security

Because we provide services in your home or community, we take extra precautions to protect your privacy. However, please be aware that:

  • Standard email and SMS (text) communications are not encrypted and carry a risk of interception.
  • Discussions in your home may be overheard by others in the household; please notify your therapist if you require a more private area for consultation.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with Mobile Rehab LLC at the address above or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
