Q: What is In-home outpatient physical therapy?

A: It is the same as a physical therapist who works in a clinic making a housecall to see the patient at their home. It is also billed exactly the same way. (under Medicare Part B regulations)

We work with community dwelling seniors who are trying to remain independent at home. Since they are not home-bound, this means that they do not qualify for traditional "Home Health" therapy services. (which are billed under Medicare Part A)

The vast majority of challenges that they face, however, are in their home environment. Therefore, is is a huge benefit to be able to instruct patients in home exercises and perform home safety evaluations in their actual home! (...as compared to trying to talk to a patient about it in a clinic setting, but not actually knowing what they deal with at home!)

Q: Is this allowed by Medicare?

A: Yes. It has been allowed for a long time, too. Here is what the Medicare Benefits and Policy manual says about it.

Q: Is this allowed by other insurances?

Yes. Other Medicare replacement/ Advantage plans also follow Medicare regulations. Here is United Health Care's stance on in-home out-patient PT.

In addition, Elise Latawiec, PT, MPH, the APTA practice management senior specialist, had this to say about providing in-home, out-patient physical therapy. (She refers to hospitals, but the same holds true for out-patient PT providers.)

Q: Why don't more clinics/providers offer this service?

Because you can see more patients in a shorter amount of time if they all come to you, instead of going to them. For seniors trying to remain living independently in their homes, however, transportation can become a significant barrier to receiving the therapy they need to remain independent. That is why we will continue to build a model where physical therapists can help seniors in their own communities remain strong, active, and independent with in-home out-patient physical therapy!

I have been in physical therapy for over four months and have four torn muscles in my rotator cuff. In the beginning my physical therapist, Kelly Swimm, started out very gently.
